SURVEY CONTEST (the "Contest")
OFFICIAL CONTEST RULES

  1. No purchase necessary. A purchase will not increase your chances of winning.

  2. DEFINITIONS: In these Official Contest Rules (the “Rules”), an Eligible Person (defined in section 4) who enters the Contest is called an “Eligible Entrant.” Loblaws Inc. is called the “Sponsor.” Ipsos Reid (“Ipsos”) will be referred to as the “Contest Administrator.” Contest commencement date is 6:00 AM on 1 January 2013. Contest closing date is 11:59:59 PM on 4 January 2014. All references to times in these Rules are Eastern Standard Time (”EST”). The Contest commencement date and Contest closing date are for overall design purposes only and are NOT the relevant dates for completing the official Survey entry form which is based on the twelve (12) Contest Segments below. The twelve (12) Contest Segments will start and end on the following dates:

    Segment 1: 6:00 AM on 1 January 2013 to 11:59:59 PM on 31 January 2013
    Segment 2: 6:00 AM on 1 February 2013 to 11:59:59 PM on 28 February 2013
    Segment 3: 6:00 AM on 1 March 2013 to 11:59:59 PM on 31 March 2013
    Segment 4: 6:00 AM on 1 April 2013 to 11:59:59 PM on 30 April 2013
    Segment 5: 6:00 AM on 1 May 2013 to 11:59:59 PM on 31 May 2013
    Segment 6: 6:00 AM on 1 June 2013 to 11:59:59 PM on 30 June 2013
    Segment 7: 6:00 AM on 1 July 2013 to 11:59:59 PM on 31 July 2013
    Segment 8: 6:00 AM on 1 August 2013 to 11:59:59 PM on 31 August 2013
    Segment 9: 6:00 AM on 1 September 2013 to 11:59:59 PM on 30 September 2013
    Segment 10: 6:00 AM on 1 October 2013 to 11:59:59 PM on 31 October 2013
    Segment 11: 6:00 AM on 1 November 2013 to 11:59:59 PM on 30 November 2013
    Segment 12: 6:00 AM on 1 December 2013 to 11:59:59 PM on 31 December 2013

  3. HOW TO ENTER: Each person that successfully completes the Sponsor’s survey (the “Survey”) either online at www.storeopinion.ca (“Sponsor Website”) or via Interactive Voice Response (“IVR”) by calling 1-877-234-2322 during a given Contest Segment (defined in section 2 above), will be automatically entered into the draw held for that Segment. Alternatively, you may enter by printing your name, e-mail address, complete mailing address, and telephone number, and the words “Loblaws Inc. Customer Satisfaction Survey" on a plain 3"x 5" piece of paper and mailing it in a sealed, stamped and hand printed #10 envelope to: Loblaw Companies Limited Customer Satisfaction Survey, Ipsos Reid, 160 Bloor Street East, Suite 300, Toronto, Ontario M4W 1B9. All mail-in entries must be postmarked by the end of a Contest Segment to be eligible for that Segment, and received no later than the 10th of the month following the last day of the Contest Segment. The Contest Sponsor is not responsible for delays due to postal strike or other causes that result in late delivery of ballots mailed before the end of each Segment. Limit of one entry per outer stamped envelope. Entries are not cumulative. To be eligible for the draw in each Segment, you need to complete a Survey during each Segment or mail-in a 3” X 5” entry as outlined above. Incomplete, illegible, false, damaged, misdirected, delayed, altered or mutilated entries will be disregarded. A human response is required - any use of automated devices for the purposes of entering the contest will automatically disqualify the Eligible Entrant. If the Eligible Entrant participates in the Survey via online, normal Internet access and usage charges imposed by your on-line service and/or telephone provider may apply. The Sponsor and Contest Administrator further reserve the right in their sole discretion to disqualify from the current or future draws any Eligible Entrant who tampers with the operation of the Contest or violates the Rules. Any attempt by any person to deliberately damage any website or undermine the legitimate operation of the Contest will result in disqualification. Any such action may be in violation of civil and criminal laws, and Sponsor and Contest Administrator reserve their rights to seek damages from any such person to the fullest extent permitted by law.

  4. ELIGIBILITY: In order to enter the Contest, you must meet the following requirements:

    1. Eligible Entrant must be a legal resident of Canada at the time of entry and at the time the prize is awarded.
    2. Eligible Entrant must be the age of majority in the province or territory in which they reside at the time of entry.
    3. Employees of the Sponsor and Contest Administrator and/or their subsidiaries, affiliates and related companies, and members of their households (whether related or not) and immediate families (defined as spouse, parent, child, sibling) are not eligible to participate. Employees of agencies that provide advertising, promotion and fulfillment services to Ipsos, as well as affiliates and operators of affiliate websites including members of their households and immediate families, are not eligible to participate.

    Contest Administrator reserves the right to remove an individual from this Contest for any violation of these Rules, including, but not limited to, if the Contest Administrator has a reasonable suspicion that an individual has provided false or dishonest responses to any component of the Survey.

  5. PRIZES: The number of prizes available to be won in each Contest Segment are as follows:

    The total retail value of the prize draws is $6,000 per Segment, for a total prize value of $72,000. Prizes will be paid in the form of a cheque made payable to the winner. Sponsor or Contest Administrator will not be responsible for replacing any prize that is lost, stolen or misplaced.

  6. PRIZE DRAWING DETAILS: Eligible Entrants will be entered into a draw in which one (1) entrant will be chosen for each available prize per Segment, by a random computerized draw based on entries received during the Contest Segment. This selection will be done at the end of each Contest Segment in the Contest Administrator’s offices located at 160 Bloor Street East, Suite 300, Toronto, Ontario M4W 1B9, Ontario on the following dates and times:

    Selected entrants will be notified via telephone or email within four (4) business days of the draw at the telephone number or email address provided at the time of entry (the “Notification”). Before being declared a winner, each selected entrant will be required to sign and return a ‘Release and Declaration’ form (as described in more detail below) and to correctly answer, without assistance of any kind, whether mechanical or otherwise, a mathematical skill-testing question. The ‘Release and Declaration’ form, including the skill-testing question will be emailed or mailed to the winner and must be completed and returned to the Contest Administrator’s office within five (5) business days after receiving the ‘Release and Declaration’ form. Should the Contest Administrator not receive a response to the Notification within five (5) business days, or should the selected Entrant incorrectly answer the skill-testing question or otherwise not comply with the Rules, the prize will be forfeited and the selected Entrant will no longer be considered eligible to win a prize in the applicable Segment. Once the selected Entrant has fulfilled all the obligations under the Rules, the prize will be mailed to the selected entrant. Winners should allow four to six weeks from receipt of the Declaration and Release for their prize to be delivered to them. See section 7 for details on additional drawings. The odds of winning are dependent on the total number of entries received in each Segment. Prior to being declared a winner, the selected Entrant must be verified by Sponsor and Contest Administrator. Sponsor and Contest Administrator reserve the right to require proof of age and jurisdiction of residence prior to awarding any prizes. Cheques will be issued in the name of the selected Entrant; prize is not transferable. The decisions of Contest Administrator and/or Contest Sponsor on all issues relating to the Contest will be final and binding on all entrants.

    The Release and Declaration will be in a form satisfactory to Sponsor and Contest Administrator and will confirm compliance with the Rules, acceptance of the prize and will permit the Sponsor and Contest Administrator and their affiliates and subsidiaries to use the winner’s name, photograph and/or video image in any publicity and advertising without compensation or notice, unless otherwise prohibited by law and, in addition, will release the Sponsor, its affiliates and related companies, franchisees, advertising and promotional agencies and the Contest Administrator and all of their respective agents, officers, directors, employees and representatives (the “Releasees”) from any and all liability, damages, costs, expenses and claims arising out of or relating to the entrant’s participation in this Contest. In the event that the potential winner fails to sign and return such form within the time period indicated on the form, the Entrant will be deemed to have forfeited the prize.

  7. DRAWING FOR FORFEITED PRIZES: If for any reason, whether it be forfeiture or otherwise, a Contest prize is not awarded to a selected Entrant, a second list of selected entrants will be used. At the time of the initial randomized computer draw, 13 extra names will be selected per Segment. If the initial selected entrants have not claimed their prizes within 30 days from the end of the applicable Segment, this secondary list will be used, in order, to replace the vacant prize winners. The new selected Entrants will required to complete the process outlined above for the original selected Entrant to claim their prize. Should these selected entrants fail to claim their winnings within the allotted time period, additional winners will be selected from the remaining pool of Eligible Entrants in that Segment who will follow the same process of elimination and verification until all prizes are awarded.

  8. LIMITS OF LIABILITY: Sponsor and Contest Administrator have no responsibility whatsoever for injuries, losses, or damages of any kind that result from acceptance, possession, or use of any prize. Sponsor and Contest Administrator assume no liability for lost or misdirected entries or Surveys, whether due to technical or computer malfunctions or otherwise. Sponsor and Contest Administrator will not be liable for technical or computer malfunctions of any kind. If, for any reason, the Contest is not capable of running as planned, including infection by computer virus, bugs, tampering, unauthorized intervention, fraud, technical failures, or any other causes beyond the reasonable control of Sponsor and Contest Administrator, which corrupt or affect the administration, security, fairness, integrity or proper conduct of the Contest, Sponsor reserves the right at its sole discretion, subject to the approval of the Regie des alcools, des courses et des jeux, if necessary, in the province of Quebec, to cancel, terminate, modify or suspend the Contest.

  9. CHANGE OF RULES: These Rules are subject to change (subject to the approval of the Régie des alcools, des courses et des jeux, if necessary, in the province of Quebec), if necessary, and any changes to the Rules will be posted on the Sponsor Website and in-store at the customer service desk and become effective immediately upon such posting.

  10. RELEASE: By entering the Contest, each Entrant agrees that (i) Sponsor and Contest Administrator may contact that Entrant by e-mail, phone or postal mail and that the Entrant’s personal information will be used for the purpose of administering the Contest; (ii) if Entrant is a winner of a prize, Sponsor and Contest Administrator may use that Entrant's entry, name, likeness, photograph, and any statements regarding the Contest attributed to the winner, for editorial, public relations, promotional, and advertising purposes on behalf of Sponsor and Contest Administrator in all media worldwide, without any obligation, notice or compensation to winner, except where prohibited by law; (iii) to release Sponsor and Contest Administrator, and their parent-companies, affiliates, and subsidiaries, the service agencies, and independent contractors of any of the above organizations, and their respective directors, officers, employees, and agents, including advertising and promotion agencies, from any and all liability, loss or damage arising out of their participation in the Contest and with respect to the awarding, receipt, possession, use and/or misuse of any prize; and (iv) to sign a document to this effect.

  11. WINNERS LIST: For a list of winners from previous Contest Segments, please send your request to the following: Loblaws Inc. Customer Satisfaction Survey Winners, Ipsos Reid, 160 Bloor Street East, Suite 300, Toronto, Ontario M4W 1B9.

  12. APPLICABLE LAW: The Contest is subject to all applicable federal, provincial, and local laws and regulations. The Contest is governed by the laws of the province of Ontario and, unless indicated otherwise in these Rules, all claims arising from or relating to the Contest shall be resolved in the provincial or federal courts located in Ontario.

  13. RESIDENTS OF QUEBEC: Any litigation respecting the conduct or organization of a publicity contest may be submitted to the Regie des alcools, des courses et des jeux for a ruling. Any litigation regarding the awarding of a prize may be submitted to the Regie only for the purpose of helping parties reach a settlement.
